When should I arrive for my appointment?

You should arrive at least 15 minutes prior to your scheduled appointment time.  You will need to fill out paperwork and parking can sometimes be tricky.  If you would like to save some time, please download our new patient information sheet, and have it filled out when you come in to see us.  Click here to download our new patient form.

Will I be responsible for making any payments the day of my visit?

you will be responsible for your co-pay or a percentage of your deductible. that information is usually  available by calling the customer service number located on the back of your insurance card.

health insurance information, picture i.d., and health history (i.e., previous surgeries)

the medical assistant to the doctor that will be performing your surgery.
